Homeless Services

Individuals who are experiencing homelessness and in need of Upward Transitions’ assistance should discuss the need for a referral to us with their case manager, advocate, or counselor at the shelter or transitional housing where they reside or at the agency providing them with healthcare, mental healthcare, rehabilitative services, etc. for:

  • Legal document retrieval assistance for individuals experiencing homelessness (birth certificates, marriage licenses, etc.)
  • State ID purchase assistance for individuals experiencing homelessness
  • Public housing rent deposits (when funds are available)
  • Linkage to other services and referrals to our partner agencies

Verification

An Upward Transitions’ partner agency must email our Verification of Homelessness and Referral form to us for any individual seeking the following services:  birth certificate retrieval, obtaining State ID, and transportation for employment purposes.

Ways to obtain a referral:

  • If you reside in a shelter, please contact your case manager at the shelter for a referral.
  • If you reside on the streets or in an abandoned building, tent, or car, please contact an agency, day shelter, or service provider that can verify your homeless status and provide a referral to us.
  • If you reside in a transitional housing program, please contact your advocate in the program for a referral.
  • If you reside with others temporarily (1-2 months not paying rent), please contact an agency, day shelter, or service provider that can verify your homeless status and provide a referral to us.
  • If you have recently been released from incarceration, please contact your case manager, parole officer, or advocate for a referral.
