Please note, depending on the number of requests we receive, it may take up to 48 hours before you are contacted. We will make every effort to keep you informed, but please understand, the need in our community is great and we are working diligently to help as many people as we can.

Once Upward Transitions receives your request for assistance, you will be contacted by a case manager to discuss your request for assistance. If you appear to meet the basic eligibility requirements for assistance, you will be asked to provide the following documents to complete the application process.

Please note, some programs may require additional documentation.

  • Completed Agency Application.
  • Signed Authorizations for Release of Information for property owner/manager and/or utility service providers.
  • Copy of drivers license or state identification card and Social Security cards for adult family members.
  • Copies of Social Security cards for family members or alternate documents to verify Social Security numbers/children in the home.
  • Copy of lease if requesting rent assistance.
  • Copy of 5-day notice to quit or eviction notice if requesting rent assistance.
  • Copy of utility bill(s) with cutoff notice if requesting assistance with utility bill(s).
  • Proof of hardship which has caused difficulty in paying the bill. Loss of income after March 1, 2020 due to the impact of COVID qualifies as a hardship.
  • Documented verification of household income 
  • Completed Property Owner/Manager Verification form.
  • Completed W-9 from the property owner/manager.
