Upward Transitions Homeless Prevention Case Manager

Educational Requirements:

Bachelor’s degree in social work or related field of study from an accredited college or university.

Experience Required:  2-3 years experience in the social service field

Basic Function:

The Homeless Prevention Case Manager provides services to families and senior who are facing eviction to prevent homelessness by focusing on short-term financial assistance, case management advocacy, property manager/owner mediation and linkage to other supportive services. Reports to Program Director.

Knowledge, Skills, & Abilities Required:

  • Working knowledge of principles and methods of interviewing
  • Knowledge of social and economic issues creating poverty, working successfully with practices and techniques related to people with low income to achieve greater self-sufficiency
  • Knowledge of available community resources
  • Skill in handling participant behaviors such as fear, hostility, aggression, etc.
  • Ability to be culturally sensitive to diverse participant populations
  • Ability to work harmoniously with applicants, the general public, partner agencies, property managers/owners, utility companies, and other employees
  • Ability to maintain accurate and systematic records, and to manage workload to meet deadlines
  • Ability to use automated technology to establish and maintain case records
  • Ability to explain and apply program policies and procedures

Key Responsibilities:

  • Conduct an intake assessment to determine a participants’ eligibility for services.
  • Develop a housing stabilization plan with participants.
  • Assist participants with the development of and encourage adherence to a personal budget plan to assist participants in maintaining their housing.
  • Inspect housing to ensure it meets housing habitability standards before financial assistance is provided, when required by funding.
    • Submit requests for short-term housing assistance including rent and utility payments and housing deposits in a timely manner.
  • Provide assistance with additional services such as bus passes and gas cards for travel to work, social service appointments, medical appointments, and general needs; identifying document and State ID retrieval; or additional basic needs when supplies are available.
  • Provide information to participants regarding options available to them within the social service and community resource systems and refer, as appropriate, to services which will meet their needs.
  • Collaborate with partner agencies to assist participants in accessing support services.
  • Advocate for participants to secure needed services and resources for which they are eligible.
  • Maintain contact with the participant and offer ongoing housing-focused case management services to assist the participant in maintaining permanent housing, when required by funding.
  • Respond to calls, emails, texts, letter update requests, and other inquiries in a professional and timely manner.
  • Maintain accurate and complete documentation of current service activities using Upward Transitions procedures, forms, and reporting systems in a timely manner to ensure required performance measures are tracked.
  • Keep complete and orderly files for efficient retrieval of information.
  • Protect participant confidentiality by keeping participants’ case files in designated file cabinets and only release information when the participant has signed an authorization for release of information that is time-limited and specifically states what information can be released.
  • Assist in the collection of data and preparation of program reports.
  • Participate in company, community, and partner meetings, training, and events when requested.
  • Other duties as assigned and required.

Must have valid driver license, reliable transportation, minimum legally mandated auto insurance, and pass background check and pre-employment drug screening.  

This position usually works in a typical office setting at Upward Transitions as well as occasionally visiting participants in their homes, place of employment, or partner agency. There may be extended periods of sitting, standing and/or bending; listening, talking, and/or visual concentration; writing and/or computer use. Lifting should not normally exceed 50 pounds.
